The following over-the-counter medications may be administered to your child if needed, all dosed per label instructions by age/weight. Please consent by typing initials below.

Trip Permission: My son/daughter has permission to engage in all prescribed event/trip activities which may include, but are not limited to, water sports, snow skiing, paintball, and other rigorous indoor and outdoor activities, unless otherwise noted by me, the parent or legal guardian. My son/daughter has permission to leave the activity grounds for activity-related outings and purposes. I give consent that my child may be transported in any vehicle designated by Northwood Baptist Church Student Ministry Staff or Volunteer Leader in whose care he/she has been entrusted while attending and participating in sponsored activities. By completing this form, I agree that Northwood Baptist Church has my permission to use my or my children's photograph publicly to promote the church. I understand that my child's photo may be taken, and the images may be used in print publications, presentations, websites, and social media. I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

Property Damage: Northwood Baptist Church is a non-profit charitable organization dependent on God and his people. Those who engage in any related activities waive and release Northwood Baptist Church from any claim for personal injury, illness, or property damage. Attendees agreee to carry insurance and/or cover the expenses related to personal injury, illness, or property damage.

Health/Policies: I understand that all medicines, vitamins, etc. must be given to the Northwood Baptist Church staff representative upon check-in and that they must be in their original containers.No medication may be given without being listed above and signed by a parent/guardian. Illegal drugs, weapons, and similar items are not permitted at any Northwood Baptist Church activity. Northwood Baptist Church reserves the right to search for and remove such items from anyone suspected of possessing them.

Medical Emergencies: I hereby give permission to the medical personnel selected by Northwood Baptist Church staff representative or youth volunteer to order x-rays, routine tests, and treatment for my son/daughter. In the event I cannot be reached, I hereby give permission to the physician selected by Northwood Baptist Church staff representative or youth volunteer to hospitalize, secure proper treatment for, and order injection, anesthesia, and/or surgery for my child as named below.

I, _______________________, agree and understand that by signing the Electronic Signature Acknowledgment and Consent Form, that all electronic signatures are the legal equivalent of my manual/handwritten signature and I consent to be legally bound to this agreement. I further agree my signature on this document is as valid as if I signed the document in writing. This is to be used in conjunction with the use of electronic signatures on all forms regarding any and all future documentation with a signature requirement, should I elect to have signed electronically.
